چکیده
Facial paralysis is a devastating condition with a severe emotional impact on patients as it significantly affects appearance and impairs function. Patients complain of not being able to hold food and liquid in their mouth, not being able to speak clearly, and not being able to smile. T h e i n a b i l i t y t o s m i l e i s o f t e n psychologically troubling to patients. This presents a challenging problem for reconstructive surgeons as no single technique can be used to correct this ailment. Rather, a comprehensive plan tailored to the patient and utilizing a variety of procedures must be employed. In cases of paralysis of a short duration (< 1 year), direct reinnervation techniques provide the best result. However, in cases where longstanding facial paralysis is present, this is no longer possible as muscle atrophy and nerve end plate fibrosis would have occurred.2 Options in these cases are limited to either regional muscle transfer3, in which an adjacent muscle is simply transposed to provide movement, or free tissue transfers, in which a distant muscle is completely removed, transposed, and reinnervated.

Principles and biomechanics of muscle tendon unit transfer: application in temporalis muscle tendon transposition for smile improvement in facial paralysis.
Muscle tendon unit (MTU) transfer is a common procedure performed to restore hand function after peripheral nerve or spinal cord injury. The principles of MTU transfer established for hand surgery can be adopted to optimize the dynamic excursion of the temporalis tendon transfer procedure for facial reanimation. Additionally, the force generating ability of a transferred MTU depends on the idea...

Long-standing peripheral monolateral facial paralysis in the adult has challenged otolaryngologists, neurologists and plastic surgeons for centuries. Notwithstanding, the ultimate goal of normality of the paralyzed hemi-face with symmetry at rest, and the achievement of a spontaneous symmetrical smile with corneal protection, has not been fully reached. At the beginning of the 20(th) century, t...

OBJECTIVE To compare objective outcomes and complications following temporalis tendon transfer in patients with and without a history of radiation to the parotid bed. METHODS Retrospective medical chart review comparing dynamic movement of the oral commissure and resting symmetry achieved in 7 irradiated patients (group R) and 7 nonirradiated patients (group N) after temporalis tendon transfe...
متن کاملTemporalis muscle-galea pedicled flap for reconstruction of longstanding facial paralysis.
Reanimation of longstanding facial paralysis is a difficult clinical problem commonly tackled with the method of pedicled muscle flap transfer. The temporalis muscle has been the most popular. In the past, one common problem was that the flap was not long enough to reach parts of the face distant from the affected area. To overcome this disadvantage, we have devised a flap consisting of the ped...

OBJECTIVE To describe a minimally invasive approach of the temporalis tendon transposition technique for dynamic reanimation in patients with long-standing facial paralysis. METHODS We report a case series of 17 consecutive patients with facial paralysis who underwent minimally invasive temporalis tendon transposition surgery for dynamic facial reanimation between January 1, 2006, and Decembe...
